of the 2006 form from the IRS website by following the below link. 2006 IRS Tax Form #5695 (http://www.irs.gov/pub/irs-pdf/f5695.pdf) My understanding is that there shouldn't be any major changes in the 2007 form, so this should give you a good idea of the information you will need in order to complete the form. For your reference, last year's form was released Jan 03 of this year. I would assume the 2007 form will be released on a similar schedule.
