IRS Form #5695 is not yet available for 2007, but you can print off a copy
of the 2006 form from the IRS website by following the below link.

My understanding is that there shouldn't be any major changes in the 2007 form, so this should give you a good idea of the information you will need in order to complete the form.  For your reference, last year's form was released Jan 03 of this year.  I would assume the 2007 form will be released on a similar schedule.
